Uses evidence-based engagement strategies including assertive engagement, motivational interviewing, and other techniques that seek to develop meaningful helping relationships with clients.
Develops written bio-psycho-social assessments to identify strengths and needs of families.
Develops a service plan in collaboration with the family that identifies goals and tasks related to increasing self-sufficiency and well-being for both the parent and the child as well as the family as a whole; helps clients realize milestones and long-term goals.
Identifies and links clients to community resources, transports clients to appointments and provides other supports in accessing those resources.
Provides conflict resolution and therapeutic support to clients inclusive of helping to resolve ongoing family conflict and unresolved trauma.
Participates in the enrollment of new clients into the program to include coordinating interviews, collaborating with partnering provides, assisting with identifying various move-in resources such as furniture, enrolling children into school, etc.
Conducts school visits to check in on the school performance.
Documents all client interactions in Housing the Homeless Database with thorough case notes, case reports, and case management plans within time frames.
Participates in case conferences, case reviews, and clinical supervision processes to ensure a high quality of services for PSH clients.
Works collaboratively with the Program Manager and other staff to provide services to all clients.
Assists the Program Manager with reporting and various administrative tasks.

\nMinumum Position Requirements:

Bachelor’s degree

\n \n\n

Two years case management experience post bachelor’s degree

\n \n\n

Must possess strong interpersonal, verbal, and written communication skills

\n \n\n

Must possess an attitude compatible with the goals and purposes of The National Center for Children and Families
