The Healthcare Services Navigator will coordinate with Black residents of Montgomery County who request referrals to healthcare providers and community resources within the BPHN Provider Network. The primary goal is to enhance access to providers in one (1) business day. The Navigator will document and monitor customer utilization and satisfaction of the wide range of health and community resources.
Essential Duties and Responsibilities:
Engage individuals who request referral to a BPHN health, dental or mental health provider, support their ability to access health professionals and navigate the health care system.
Provide one on one navigation to connect individuals to culturally tailored specialty and alternative health networks, to essential community services, and to resources that remove barriers that impact ability for individuals to receive health services and attend appointments.
Complete intake and health assessments with each individual and assist with needs based on the assessment results.
Ensure all clients apply for and/or acquire health insurance before connecting to a health professional for a health appointment.
Help individuals complete the health insurance application process.
Connect clients to health professionals who accept their health insurance coverage and aid with making appointments.
Provide follow-up regarding appointments and assess client satisfaction.
Enter all required client information into BPHN data management system in a timely manner, including but not limited to documentation of appointments scheduled and follow up appointments.
Provide appointment reminders and follow up.
Engage community organizations and build partnerships with existing County resources as referral sources.
Participate in all BPHN sponsored events and community events for which BPHN is invited as a vendor.
Performs miscellaneous job-related duties as assigned.

90% customer satisfaction survey rate.
Qualifications:
AA or BA degree and two years related experience or equivalent.
Must have a vehicle and a valid driver’s license.
Solid judgement/temperament in dealing the confidential and sensitive information and issues.
Commitment to teamwork, collaboration, and the agency’s mission to progressively serve vulnerable children and their families.
Physical Demands/Work Environment:
While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required y this job require close vision working with a computer monitor. While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate.
