Bilingual Spanish-Speaking Case Manager

Position Purpose:

The Care Manager delivers trauma-informed, culturally responsive case management services to BTW high school students and their families. This includes comprehensive needs assessments, service planning, resource navigation, short-term financial assistance, and connections to community services. The role is critical in advancing family self-sufficiency and supporting school engagement.

Essential Duties and Responsibilities:
 
  • Conducts intake assessments and co-develop care plans addressing housing, food, mental health, education, and other basic needs.
     
  • Completes County-approved, domain-specific needs assessments by the second non-crisis meeting and repeat every 3 months or upon discharge.
     
  • Develops collaborative, SMART-goal service plans with families by the third non-crisis meeting and review quarterly.
     
  • Provides ongoing, strengths-based care coordination, with a minimum of two monthly contacts per case (school year and summer).
     
  • Maintains a minimum caseload of 10 active households per month, averaged quarterly; document drop-ins and one-time services as required.
     
  • Ensures 60% of work time is spent in direct service, including documented support for non-caseload clients.
     
  • Supports uninsured, underinsured, recently immigrated, and other high-need student populations.
     
  • Offers services in person, virtually, in school, at home, or in the community based on family need and preference.
     
  • Documents all services in the County’s electronic records system within 2 business days of contact.
     
  • Stays current on County and State policy changes that impact client eligibility or service delivery.
     
  • Participates in monthly outcome tracking via County tools, including self-sufficiency outcomes.
     
  • Administers client satisfaction surveys and document results per contract requirements.
     
  • Collaborates with clinical and youth development staff to deliver holistic support services.
     
  • Maintains and respect all required confidentiality, consent, and recordkeeping standards.


Non-Essential Duties and Responsibilities:
 
  • Other duties as assigned.
 
  • Participates in all NCCF and DHHS required trainings.
 
  • Schedules appointments for students; provides transportation as needed to appointments. 

Minimum Position Requirements:
 
  • Bachelor’s degree in Social Work, Human Services, or related field
  • At least 2 years of experience in case management, family support, or school-based services.
  • Demonstrated ability to develop and monitor SMART goals and outcomes-based care.
  • Familiarity with County service systems and local resource networks.
  • Excellent organizational, documentation, and interpersonal communication skills.
  • Background clearance required by DHHS.
  • Bilingual ability strongly preferred.
  • Demonstration of cross-cultural competencies.
  • Knowledge of adolescent development.
  • An attitude compatible with the goals and purposes of NCCF.

Physical Demands/Work Environment:

While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle and feel. The employee is occasionally required to stand, walk, reach with arms, stoop, kneel, or crouch. Vision abilities include close vision with a computer monitor. Salary: $58,000 - $65,000 Annually
