Position Purpose: Assists program participants with obtaining housing and transitioning out of shelter within a 90 day period. Also provides intensive case management and wrap around services that helps program participants overcome barriers to achieving stability.
Essential Duties and Responsibilities:
Provides intensive case management to a caseload of at least 15 clients at least twice per week.
Conducts an initial assessment and completes intake packet within 72 hours of intake.
Develops an Individualized Housing Stabilization Exit Plan within 7 days of intake.
Screens clients for diversion services within the first 7 days after intake and every 7 days for the first 30 days.
Ensures that F-SPDATs are completed within 7 days of intake and updated every 90 days.
Conducts client credit checks within 14 days of intake.
Engages and builds a rapport with clients on your caseload in an effort to help them address barriers to obtaining housing such as housing, employment, mental and physical illness, etc.
Prepares monthly reports and submits them to the Case Manager Supervisor by the 1st of each month.
Ensures case notes are entered into HMIS, CATCH, and STEP within 24 hours of meeting with the clients.
Assists families in obtaining vital information, i.e. Social Security card, INS documentation and identification cards. Ensures that initial information upon admission, (i.e. signatures of legal guardian, consent forms, social security card, and birth certificate) is obtained and in the resident’s file.
Attends weekly case conferences with TCP & DHS liaison’s.
Attends weekly internal case reviews with New Beginning Temporary Family Shelter Housing team around case management and residential activities.
Ensures that client files are maintained according to Agency, District of Columbia, Federal and COA regulations.
Assist with researching and locating viable housing leads in the District of Columbia in.
Builds community partnerships with private and cooperate landlords.
Accompanies clients to view potential housing units.
Serves as a liaison between NCCF staff and other agencies providing services to the family.
Engages community resources for homeless residents, i.e. financial assistance, substance abuse treatment and support programs, trauma recovery groups, empowerment programs, etc.
Make appropriate referrals and assist clients with resources for moving out of the shelter.
Conducts 30 & 60 day follow up with clients that have exited the shelter.
Provide clients with job development services such as resume preparation, job readiness skills, weekly job leads, and locating and registering clients for vocational and educational trainings.
Completes at least 4 mandated TCP trainings monthly.
Non-Essential Duties and Responsibilities:
Advocates on behalf of homeless families in local, state and national forums.
Participates in trainings that enhance knowledge on how to effectively work with diverse clients and families
Other duties as assigned.
Qualifications:
Associate’s Degree or two (2) years of college. In lieu of the education requirement, the Contractor shall ensure the case manager has a minimum two years of experience with homeless services programs or provision of social services, as well as experience with transitioning clients into housing.
Note, the two years of experience may be waived if the candidate for employment can document having experienced homelessness.
Exceptional teamwork orientation with good written/oral communication skills.
An attitude compatible with the goals and purposes of the National Center for Children and Families.
Physical Demands/Work Environment:
While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required y this job require close vision working with a computer monitor. While performing the duties of this job, employee is occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate. While performing the responsibilities of the employee’s job, these work environment characteristics are representative of the environment the Human Resources Director will encounter.
